Scientists at 23andMe have identified specific gene variants linked to differing outcomes when taking medications like Wegovy and Mounjaro. The study, published in Nature, offers a potentially groundbreaking approach to personalized obesity treatment—though with caveats.
The research focused on 27,885 patients utilizing GLP-1 receptor agonists. Researchers pinpointed a variant, rs10305420, associated with a slight, yet statistically significant, increase in weight loss among those carrying it. Conversely, another variant, rs1800437, correlated with a higher incidence of nausea and vomiting in patients taking tirzepatide. This suggests a complex interplay between an individual's genetic makeup and their response to these increasingly popular drugs.

A modest, not definitive, influence
However, the researchers emphasized that genetics account for only a small fraction of the variability observed in weight loss responses. Marie Spreckley, an obesity expert at the University of Cambridge, noted that “behavioural, clinical and treatment-related factors appear to explain a substantially larger proportion of variability.” Essentially, the type of drug, dosage, duration of treatment, and even the patient’s sex appear to exert a far more pronounced influence than genetic predisposition.
“It’s reinforcing that while there is considerable variability in response to GLP-1 therapies, genetics is only one piece of a much more complex picture,” Spreckley continued. “We’re talking about a confluence of factors—lifestyle, environment, and the drug itself—that ultimately determine the outcome.”
The findings don’t offer a simple ‘genetic test to predict efficacy’. Instead, they highlight the need for a more holistic understanding of obesity treatment, moving beyond a one-size-fits-all approach. The work represents an important step, but the implications for routine clinical practice remain modest.
